Chronic kidney disease often causes few or no symptoms in its early stages, which is exactly why routine blood and urine tests matter more than waiting to feel unwell. This guide covers the signs and symptoms of kidney problems people are most commonly told to watch for.

Early kidney disease symptoms

In the early stages (CKD stages 1–2), most people have no noticeable symptoms at all. The kidneys have substantial functional reserve, so measurable changes can occur well before symptoms appear. This is why an eGFR and urine ACR test, not symptoms, are the reliable way to catch early kidney disease.

Signs and symptoms of kidney problems in later stages

As kidney function declines further, some people notice:

• Fatigue or low energy
• Swelling in the legs, ankles, or around the eyes
• Changes in urination (foamy urine, more or less frequent, or blood in the urine)
• Nausea or reduced appetite
• Itching
• Muscle cramps
• Shortness of breath
• Difficulty concentrating

These symptoms have many possible causes beyond kidney disease, and having one or more doesn't necessarily mean you have CKD — but they're worth discussing with your doctor, especially alongside risk factors like diabetes or high blood pressure.

Symptoms of kidney failure

At more advanced stages (CKD stage 4–5), symptoms tend to become more noticeable and can include significant swelling, marked fatigue, confusion, and reduced urine output. Severe or sudden symptoms — especially very little urine, chest pain, or severe shortness of breath — need urgent medical attention, not a wait-and-see approach.

Kidney disease symptoms in women

The underlying warning signs of kidney disease are broadly similar between men and women. Some symptoms, such as urinary tract infection patterns or changes noticed during pregnancy, may be first picked up in different contexts for women. Any new or unusual symptoms are worth discussing with your doctor rather than self-assessing based on general lists.

Acute vs. chronic kidney symptoms

Acute kidney problems (a sudden, often reversible drop in kidney function, usually from illness, dehydration, or medication) can cause symptoms that come on quickly — sudden reduced urination, swelling, or confusion. This is different from chronic kidney disease, which develops gradually. Sudden symptoms deserve urgent medical evaluation regardless of which one it turns out to be.
